In the days of OS 9, all you needed to edit an icon was a copy of ResEdit (free from Apple). Nowadays, with OS X levels of transparency and all that, you need something like http://iconfactory.com/iconbuilder.asp

The Apple Developer Tools (free with OS X or for download with free ADC registration) includes a program called Icon Composer, which creates 16px, 32px and 128px icons, including masks. There is also a program called Iconographer somewhere out there.
To edit an icon, get info on it in the Finder, click the icon and copy it to Photoshop. There are plugins out there that will export a Photoshop image to an icon, or you can copy your edited image to one of the previously mentioned apps.
You can use Can Combine Icons to stitch together various default icons... for example, I combined the default hard drive icon with the default burn icon for my internal hard drive, and I've made icons out of folders and various could-be-relevant icons for directories like games, downloads, etc.
